Qualification evidence requirements have been updated for applications for general registration.

Applicants for registration who have not completed a qualification on the Approved Programs of Study list are requested to submit certified copies of both their academic transcript and certificate. Modifications have been made to the AGEN-86 Application form for general registration to reflect this requirement.

Practitioners who have already submitted their application form or are in the process of completing their application form are not required to complete a new form. However, if you have not yet submitted your application form please attach both documents.
